How Do I - See my records

In line with the Access to General Practitioner Records Pursuant to Data Protection Act 1998, Greenswood Surgery has a procedure in place for those patients who wish to view their medical records held at the Surgery. (Please note that this act only applies to entries recorded after 1 November 1991).

If you wish to view your medical records, please contact the surgery where the Practice Manager will be happy to assist.
